The purpose of this assignment is to design a power point presentation for a patient you have seen in practicum with one diagnosis that has been discussed and presented in Unit 1 – Unit 3.

ALZHEIMERS DISEASE

Directions:

1. Provide a brief overview of the pathophysiology of the selected diagnosis.
2. Provide the epidemiology and causes of the selected diagnosis.
3. Provide the incidence and prevalence for this diagnosis in the adult patient in primary care.
4. Provide the patient’s subjective and objective clinical presentation in your practicum setting.
5. List three differential diagnoses for this selected clinical presentation.
6. Describe any diagnostic tests or laboratory testing you ordered for this patient.
7. Describe the plan of care (treatment plan) for the patient.
8. Provide follow up and referral information.
9. Provide patient education provided for this diagnosis.

Format

1. The Power point presentation should be 12 – 15 slides excluding the title slide.
2. There is no need for “voice over” or narration of the Power Point.
3. Speaker’s notes should be included with the Power Point.
4. References must be included
